Three bids. Formline's was the only one with a spec sheet, and the only one that mentioned compacting the base. Fifteen hundred more than the cheapest and worth every dollar.
Concrete, Done Like It Matters.
Driveways, patios and floors that are still flat, still crisp and still uncracked in year fifteen. Every pour comes with a written estimate and a spec sheet, because you should know what's under your feet.
Estimates in writing within 48 hours of the site visit. No verbal numbers.

What We Pour
Flatwork is a commodity until you look at it every day. We treat it like it's going to be looked at.
Driveways
Broom, exposed aggregate or stamped. Six inches at the apron where the trucks turn, control joints on a real layout, and a slope that actually sheds water away from the garage.
Patios & Walkways
Stamped and stained, or clean broom finish with a hand-tooled edge. Sealed so the color holds through Utah winters.
Polished & Garage Floors
Ground and polished to the sheen you want, or a flake epoxy system over a properly prepped slab. No peeling, because we prep.
Tear-Out & Replacement
We remove the cracked and heaved slab, fix the base that caused it, and pour it right. Same crew, same week.
What's In Every Formline Slab
This goes on every estimate. Compare it to the other bids. If they don't say, ask.
Base
- Excavated to undisturbed soil
- 4 to 6 in compacted road base
- Plate-compacted in lifts, not just driven over
- Vapor barrier under interior slabs

- 4,000 PSI air-entrained mix
- Fiber reinforcement in every yard
- #4 rebar 24-in grid, on chairs
- 6 in thick at driveway aprons, 4 in elsewhere
- Control joints cut at 1/4 depth on a drawn layout
Finish & Cure
- Hand-tooled edges and joints
- Cure-and-seal applied same day
- Penetrating sealer on decorative work
- Keep-off period in writing

A heaved and cracked 1988 driveway in Sandy. Torn out, base rebuilt and compacted, poured in a day, back in the garage in a week.

A few from the last few weeks. Every one has a spec sheet on file.

What Homeowners Say
The old driveway heaved every winter. They dug out the base and rebuilt it. Three winters later, not a crack.
The joint layout was drawn on the estimate. Little thing, but it told me they'd thought about how it would look, not just how fast they could pour it.
